By completing this form you agree to the following: I agree to send my child to Shishu Shibir and accept the rules and regulations. To the best of my knowledge and belief, I consider my child to be medically fit to participate in the activities outlined. This event will take place under the guidance of Shishukunj’s adult volunteers. I understand that, while Shishukunj’s adult volunteers are required to take all reasonable care of the children, I confirm that Shishukunj and Shishukunj’s adult volunteers cannot be held liable for any loss, damage, or injury suffered by my child or his/her possessions whilst at Kendra or in attending any Shishukunj events or during transport to and from a Shishukunj activity (where such transport has been organised by Shishukunj or any of its adult volunteers) unless such loss, damage or injury is directly attributable to negligence, wilful misconduct or fraud on the part of Shishukunj or its adult volunteers.
I agree that, in the case of medical emergencies, qualified first aiders or doctors can administer first aid, treatment or medication to my child. I understand the need for my child to behave in an appropriate manner and that Shishukunj’s adult volunteers may discipline my child appropriately during the course of activities, and may contact me/us in the event of any serious disciplinary issues arising.
